Self-employed professional Elena Krechko, Taxpayer Identification Number 773474723424, hereinafter referred to as the “Provider,” publishes this service agreement (the “Agreement”) for the provision of psychological services to individuals (the “Client”).
1. GENERAL PROVISIONS
1.1. This Agreement constitutes the Provider’s offer to enter into an agreement for psychological services on the terms stated in this document.
1.2. Submission of a request for a consultation and/or payment for services constitutes acceptance of this Agreement.
1.3. The agreement is considered concluded from the moment of acceptance.
2. SUBJECT OF THE AGREEMENT
2.1. The Provider delivers psychological consultations in person and/or remotely, including through messaging services or video communication, by prior appointment.
2.2. The available services, fees and duration are stated on the Provider’s website and/or in an individual agreement with the Client. Standard sessions are 90 minutes unless otherwise agreed. Fees may depend on the Client’s country of residence, needs, and whether the format is individual or group.
3. RIGHTS AND RESPONSIBILITIES OF THE PARTIES
3.1. The Provider agrees to:
— provide services with due professional care and in accordance with applicable professional standards;
— protect the confidentiality of information received during work with the Client, subject to legal and ethical exceptions;
— comply with applicable ethical and professional requirements.
3.2. The Client agrees to:
— provide accurate information when arranging a consultation;
— pay for services on time;
— communicate respectfully and comply with the agreed conditions of work;
— inform the Provider of relevant health or safety concerns that may affect the suitability of the service.
4. FEES AND PAYMENT
4.1. Fees are stated on the Provider’s website or agreed individually and may be updated.
4.2. Payment may be made to a designated bank account, payment card or by another method identified by the Provider.
4.3. If a Client cancels less than 24 hours before the scheduled consultation, the payment may be non-refundable except in circumstances agreed with the Provider or where applicable law requires otherwise.
5. RESPONSIBILITY AND LIMITS OF SERVICE
5.1. The Client remains responsible for personal decisions and actions taken during or after consultations.
5.2. Psychological counselling, psychotherapy and coaching do not replace emergency, medical or psychiatric care. The Client should seek an appropriately qualified healthcare professional when such care is required.
5.3. No specific therapeutic, personal, professional or financial result is guaranteed.
6. TERMINATION
6.1. The Client may stop using the services at any time. Where possible, the Client should notify the Provider at least 24 hours before a scheduled consultation.
6.2. The Provider may decline or discontinue services when professional boundaries are repeatedly violated, the agreed conditions are not followed, the requested service is outside the Provider’s competence, or a different level of care is required.
7. CONFIDENTIALITY
7.1. Information received during consultations is treated as confidential.
7.2. Confidentiality may be limited where disclosure is required or permitted by applicable law or professional obligations, including circumstances involving a serious risk of harm to the Client or another person.
7.3. The exact legal and professional rules may depend on the Client’s location and the jurisdiction governing the service.
8. OTHER TERMS
8.1. The Provider may amend this Agreement by posting an updated version on the website, subject to applicable law.
8.2. By submitting a request and paying for services, the Client confirms acceptance of the applicable terms.
8.3. Services are available only where the Provider is professionally permitted to offer them. Availability may vary by country, region and service type.
